ANNUAL Chaldean H.S. & College Scholarship/Award
and Graduation Party 2007

The Knights of Columbus/ Mar Toma Council at St. Peter Chaldean Cathedral is proud to honor our high school and college graduates of the class of 2007.

If you are a graduate of the class of 2007, complete this form and attach a recent photo to be included in the graduation yearbook. Include your score and your post graduation planning. The information will be published exactly as you provide it in this form excluding the address and the phone number.  

The cutoff for submitting graduation book application is Sunday, June 10, 2007

BASIC PARTICIPATION REQUIREMENTS

1. Must be of Chaldean heritage.

2. Must be a 2006/2007 academic year graduate from a high school or college

3. Must submit a completed application by Friday, June 10, 2007 with a wallet- size photo (pencil your name and phone number on the back of the photo).

 

I. ACADEMIC SCHOLARSHIP (HIGH SCHOOL ONLY)

1. Must meet basic eligibility requirements.

2. Submit highlights of your personal biography, including College you plan to attend and plan to major in (not to exceed 75 words)

3. Scholarship evaluation is based on 100 awarded points distributed as follows:

a. Academic Achievement (an endorsed copy of Transcript) ...65 points

b. Scholastic Assessment Test (SAT) score................................25 points

c. Leadership activities (Awards, Community Services, etc)...10 points

 

II. NEWCOMERS’ SCHOLARSHIP (HIGH SCHOOL ONLY) 

1. Must meet basic eligibility requirements.

2. Less than 5 years residency in the USA

3. Submit highlights of your personal biography, including College you plan to attend and plan to major in (not to exceed 75 words)

4. Scholarship evaluation is based on 100 awarded points as follows:

a. Academic Achievement (an endorsed copy of Transcript) ...65 points

b. Scholastic Assessment Test (SAT) score...............................25 points

c. Leadership activities (Awards, Community Services, etc)…10 points

 

III. LEADERSHIP AWARD (HIGH SCHOOL AND COLLEGE)

1. Must meet basic eligibility requirements.

2. Submit your personal biography, including recognition letters not to exceed 75 words.

3. Leadership award is based on 100 awarded points distributed as follows:

a. Leadership activities (Awards, community services, sport etc.)...65 points

b. Academic Achievement (an endorsed copy of Transcript) ...25 points

c. Scholastic Assessment Test (SAT) score.................................10 points

 

IV. ATHLETIC AWARDS (HIGH SCHOOL AND COLLEGE)

1. Must meet basic eligibility requirements.

2. Submit your personal biography, including recognition letters not to exceed 75 words.

3. Athletics award is based on 100 awarded points distributed as follows:

a. Athletics activities (Sport)......................................................65 points

b. Academic Achievement (an endorsed copy of Transcript) ...25 points

c. Scholastic Assessment Test (SAT) score...........................................10 points

We recommend purchasing tickets in advance (Tables will be numbered)

$30/person,   $300 per table (10 tickets)
